Ored Health Network is committed to protecting the privacy of your protected health information (PHI). We are required by law to maintain the privacy of PHI, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.
How we may use and disclose your information
We may use and disclose your PHI for treatment, payment, and health care operations:
- Treatment: sharing your information among the providers in your care team, including referrals to in-network specialists.
- Payment: billing your insurance plan and resolving payment questions.
- Health care operations: internal quality review, training, accreditation, and care coordination.
We may also disclose your information when required by law, in public health activities, for health oversight, or in response to lawful subpoenas or court orders.
Your rights
- Inspect and request a copy of your medical record.
- Request a correction to your record.
- Request restrictions on how we use or share your information.
- Request confidential communications (e.g., by mail to a specific address).
- Receive an accounting of certain disclosures we have made.
- Receive a paper copy of this notice on request.
- File a complaint with us, or with the U.S. Department of Health and Human Services, if you believe your privacy rights have been violated.
Contact the Privacy Officer
To exercise any of these rights, message your care team through the patient portal or submit a request via our contact form. The Privacy Officer reviews and responds to all requests within 30 days.
Changes to this notice
We reserve the right to change this notice and to make the new notice effective for all PHI we maintain. We will post the current notice on this page and provide a copy on request.
